Virtual Supportive Care

This program provides supportive care for people diagnosed with life-limiting or palliative conditions. RNs support clients to manage symptoms, meet goals for care and cope with changing health needs.

Please note: These programs are not a replacement for hospitalization or doctor/nurse practitioner visits. Life threatening or significant symptoms still require you to seek medical attention from a doctor or nurse practitioner.

What's Involved

Participation in the Community Virtual Care program takes only a few minutes a day. Your results and responses are automatically sent to your nurse. Depending on your condition you will:

  1. Measure your blood pressure, weight, and oxygen level every morning
  2. Answer a few questions to help evaluate your health
How the nurse helps
  1. Monitors and looks for changes in your measurements
  2. Contacts you regularly to review your results and answer your questions
  3. Sends reports and communicates concerns to your doctor or nurse practitioner
  4. Helps you to recognize when to seek care from your doctor or nurse practitioner versus when to seek emergency services
  5. Collaborates with other members of your health care team
